To set up an account:

Print out the application form below and complete including User ID, password, payment information, and system type.

Mail the completed application with payment to:

If payment is being made by credit card, applications may be faxed to 574-282-4651 or accounts can be set up via the phone by calling 574-280-4850.

Once the account is activated, set up instructions will be provided via mail, e-mail, fax, and are also avaiable on this site.

The $2.00 setup fee is non-refundable. If you wish to terminate your account, you must notify the Director prior to the account expiration or you will be charged for the next quarter.
